How can painful wound care be made easier when injectable anaesthesia is impractical and opioids create additional limitations? This Trauma Club session explores a topical spray-and-gel formulation combining lidocaine, bupivacaine, epinephrine and cetrimide.
The speakers discuss first-in-human data from 24 patients with traumatic lacerations, including rapid onset of anaesthesia, prolonged effect and safety monitoring. The conversation then moves into the questions that matter at the bedside: wound size and depth, exposed nerve endings, contaminated wounds, antimicrobial activity, multimodal analgesia, nerve blocks, dressings and potential military use.
The most interesting part is where the evidence meets real-world surgical problems—especially painful debridement and dressing changes, large contaminated wounds and the need to bridge the analgesic gap before definitive care. Several potential applications remain open for further study.
