Decompressive Craniectomy in Traumatic Brain Injury

This educational video explains the logic behind decompressive craniectomy and why it is not simply “brain decompression” under another name.

The lecture starts with the fundamentals: intracranial pressure, the Monro-Kellie doctrine, mass effect, cerebral edema, and the mechanisms that lead to brain displacement and brainstem compression. From there, the speaker moves into the practical criteria for an adequate frontotemporoparietal decompressive craniectomy, including the required size of the bone opening, evacuation of accessible hematomas and contusion areas, temporal bone resection, and expansive duraplasty.

A major part of the session is dedicated to decision-making in traumatic and combat-related brain injury: when CT and neuroimaging are mandatory, why the operation should be performed by a neurosurgeon, how staged or incomplete procedures increase risks, and how surgical capabilities differ across Role 1–4 levels of care.
