Bony Reconstruction for Sarcoma and Osteosarcoma

Wide oncologic resection can leave defects that are too large or biologically demanding for conventional reconstruction. This lecture focuses on how vascularized bone can turn a major skeletal defect into a durable limb-salvage reconstruction.
Using a series of challenging sarcoma and osteosarcoma cases, the speaker explores vascularized fibula grafts for the humerus, femur, tibia and forearm, including the Kapena technique combining fibula with allograft. A particularly striking case demonstrates transfer of a vascularized fibular head with its epiphysis in an 11-year-old patient, aiming to preserve future growth.
The lecture also covers a vascularized iliac bone flap based on the deep branch of the superficial circumflex iliac artery, including its use for smaller defects and cortical reconstruction. Surgical videos highlight fibula harvest, vessel preservation, pedicle lengthening and flap design, while the discussion addresses buried flap monitoring, rehabilitation, long-span fixation and the critical division of responsibilities between orthopedic and plastic surgeons.
