How do you choose between a local flap, a regional option and a free flap when the defect involves the tongue, alveolus, palate, cheek and mandible? This lecture walks through a broad reconstructive spectrum using real clinical cases, including submental, pectoralis major, radial forearm, anterolateral thigh, fibula and scapular flaps.

Particular attention is given to functional reconstruction: speech, swallowing, tongue mobility, occlusion and long-term quality of life. The discussion also highlights patient-specific planning, navigation-guided reconstruction, recipient vessel selection and the practical realities of flap monitoring and postoperative mobilization.
